The furnishing, installation and testing of approximately 4,870 linear feet of gravity sewer pipe with 14 sewer manholes,including, approximately 3,805 linear feet of 20-inch nominal DR 11 and DR 17 HDPE gravity sewer pipe by pipe burst method (replacing an existing 12 inch gravity sewer), approximately 920 linear feet of 21-inch PS 46 (SDR 35) PVC gravity sewer by open cut method, including a inverted siphon under One Mile Branch, concrete siphon inlet and outlet structures, unclassified excavation, backfill, tie-ins property restoration, pavement restoration, testing, other appurtenances and other work as shown on the Construction Drawings or indicated in the Contract Specifications.

Prospective Bidders
This Addendum forms a part of the Contract Documents and modifies the original Bidding Documents dated May 16, 2014. Acknowledge receipt of this Addendum in
the space provided on the Bid Form. Failure to do so may subject Bidder to disqualification.

This Addendum consists of 2 pages detailing the following revisions:
1. Existing GEB Drawing H-11 of 14 showing approximate locations of (2) existing fuel oil storage tanks. Note that both tanks are now in place beneath ramp to GEB
Loading dock.

CHANGES TO SPECIFICATIONS:
1. Section 01 11 00 SUMMARY – Revise paragraph 1.05 “Special Restrictions”, by adding the following to subparagraph A. 1 – Access to site: “Access to site will be
strictly down Monroe Street, south of the GEB for staging of work including any required crane erection. Though it is preferable for the existing concrete barrier wall
along the east walkway to remain intact, sections of this wall may be removed and put back to aid in boiler replacement if this work is closely coordinated with UTHSC
Facilities for the location and extent of barrier removal.”

2. Section 01 11 00 SUMMARY – Revise paragraph 1.05 “Special Restrictions”, by adding the following to subparagraph A. 3 – Use of the site by the Contractor for the
Work including parking and staging and any required Owner and occupant continuing egress: “The contractor will be provided one (1) parking space along Monroe
Street for use during construction. All other parking for the Contractor’s personnel must be independently arranged by the Contractor, or worked out
in agreement with UTHSC Campus Parking Services.”

3. Section 01 11 00 SUMMARY – Revise paragraph 1.05 “Special Restrictions”, by adding the following to subparagraph A. 4 – Restrictions on work hours: “Any
required shutdown of the boiler systems in GEB related to work performed under this project, will require night and/or weekend work to be closely coordinated and
scheduled with UTHSC Facilities.”

CHANGES TO DRAWINGS:
1. Drawing S-501, STRUCTURAL DETAILS:
A. Revise sheet by omitting the “Remove Brick Only” note in “OPENING DETAIL - 4A”, and delete callout for Detail 1C on Sheet S-501. Omit “BRICK SUPPORT DETAIL -
1C”.
2. Drawing M-120, BOILER ROOM – HVAC DEMOLITON:
A. Revise sheet by removing crosshatching in east wall directly behind existing 61-1/2” O.D. Van Packer stack, and removing note that reads “Remove Existing Bricks
Only Within Knock Out Panel Up to Existing Lintel.” The crosshatching and note regarding brick and block wall demolition for louver opening to be created to the
south in same wall shall remain on the sheet.

B. Revise sheet by adding sentence to “Boxed” Note from Addenda #1 to read as: “Contractor Shall Take all Necessary Measures and Provisions for Protecting the
Existing Loading Dock Drive and Existing 30,000 Gallon Fuel Oil Storage Tank Located Beneath for the Duration of this Project. The Loading Dock Ramp Shall Not Be
Utilized for Setup and Erection of Crane Used for Boiler Replacement.”

3. Drawing M-130 – BOILER ROOM – HVAC RENOVATION:
A. Revise sheet by removing new louver located near south end of the boiler room and located in east wall directly behind existing Van Packer stack, as well as the
note that reads “10’-0” x 12’-0” Wall Louver – Blank Off with Double-Wall Sheet Metal Panel with 1” Styrofoam Insulation (Refer to Detail on Sht. M-501).” The new 12’-
0” x 12’-0” Wall Louver to be installed to the south in same wall should remain on the sheet.

B. Revise sheet by indicating that new piping discharging from Boiler #1 and Boiler #3 are 6” in size for installation of new OS&Y and Stop-Check Valves. Provide any
required pipe reducers. Indicate that new OS&Y valves at steam header are 6” in size.

4. Drawing M-501 – HVAC SECTIONS AND DETAILS:
A. For Section 1 – SECTION THROUGH BOILER ROOM, remove the leader pointing to a 10’ x12’ louver section and dashed louver section shown to be installed directly
behind the existing Van Packer Stack, since brickwork at this section of wall is to now remain in place. The note that reads “New 12’x12’ & 10’x12’ Wall Louvers
(Foreground)” should be revised to now read “New 12’x12’ Wall Louver (Foreground)”. I.E. the brickwork shall now remain in place in this location.

        A. Revise sheet by adding the following note for Boiler 1 and 3: “Provide a 4” Camphered Concrete Pad beneath Boiler, Designed to Support its Full Weight and Extending 6” Around its Perimeter.”

        B. Revise sheet by adding the following note “Boiler Safety Relief Vents from each Existing Boiler Terminate into a Larger Common Vent that Terminates into Existing Van Packer Vent Stack up to Roof.”

        A. For Detail 5 “COMBUSTION AIR LOUVER DETAIL”, revise Note 5 to read as: “For Louver Framing, Refer to Structural Sheet S-501 For Opening Details”.
